Elective cosmetic procedures are not covered by insurance; we will make payment arrangements for procedures, prior to your treatment. For patients with-out insurance coverage, we ask for payments at the time of the visit. Our practice accepts Visa, Discovery,MasterCard, and American Express. It is essential to provide our office with all of the required managed care referral and authorizations forms prior to, or at the time of your office visit.
